1. Call your insurance provider
Ask the member-services or durable-medical-equipment department:

- Which claim form and supporting documents are required?
2. Obtain a prescription
Ask your licensed healthcare provider for a prescription using the terminology required by your insurer. The diagnosis and wording must match your plan's requirements.

5. Submit your claim
Follow your insurer's submission instructions and keep copies of every prescription, receipt, approval, and claim document. If a claim is denied, ask the insurer for the reason in writing and whether an appeal is available.
